Secondary Excision of Inadequately-Treated Soft Tissue Sarcoma

Abstract
Soft tissue sarcomas are often excised with intralesional or narrow margin without thorough preoperative evaluation of the extent and nature of the tumor. Inadequately-treated soft tissue sarcoma should be dealt with some additional methods. The purpose of this study was to assess the necessity of reexcision after unplanned initial surgery. From July 1989 to September 1995, 30 patients were referred to our hospital for additional treatment of soft tissue sarcoma after inadequate surgery. Mean age of the patients was 43 years, ranging from 17 to 74. Histologic diagnosis included malignant fibrous histiocytoma in 14 patients, synovial sarcoma in 4, fibrosarcoma in 3, malignant schwannoma in 3, liposarcoma in 2, 1 and 1 each of rhabdomyosarcoma, leiomyosarcoma, dermatofibrosarcoma protuberans and Kaposi¡¯ s sarcoma. All of them underwent secondary excision with a wide resection margin at our hospital. Twelve patients were treated with postoperative radiation therapy or chemotherapy. Residual tumors were found in 21 patients[70%]. Metastasis occurred in 2 patients and local recurrence in 1. Twenty seven patients have been continuously disease free for a mean follow-up of 39 months after secondary excision. In conclusion, soft tissue sarcoma should be treated with well-planned surgery initially, and secondary excision is recommended regardless of the involvement of resection margin after unplanned excision.
